Request Medical Records

Medical records can be provided to patients who receive services through the Tanana Chiefs Conference healthcare system. Requests for medical records can be submitted via email, phone call, or fax. Please keep in mind that we are unable to provide medical records for care received at facilities outside of our system – those requests will need to go directly to those facilities.
