Family Medicine Care Teams

CAIHCWe have adopted the Care Team method for providing services to strengthen our plan towards offering high-quality, comprehensive health care for our patients. Under this method, patients are matched with a dedicated team of doctors, physician assistants, care coordinators, nurses and unit clerks, which they will visit each time medical care is needed.

The Care Team model of care follows national best practice standards and will help foster a trusting relationship between patients and medical providers. Every attempt will be made to schedule an appointment with your primary care provider, or a member of their care team. Providers are available for same day appointments, which can be scheduled by contacting the Care Team’s extension or by contacting your Care Team’s Care Coordinator.

If you have after hours emergencies or medical concerns please contact our answering service and the Providence Hospital Triage Phone Nurse at 451-6682 or 1-800-478-6682

For urgent follow-up appointments deemed necessary by a medical provider, or to assist in travel preparation from the villages, our staff will offer a limited number of advance appointments. Our main goal is to provide same day access; therefore, we would like patients to call on the day they would like to be seen.

Patients are encouraged to become familiar with your Primacy Care Provider and their Care Team. Please free to contact us and visit us while you are in Fairbanks.

There are three Care Teams, and the Urgent Care Clinic, who are dedicated to working together to help you get the best possible care you need, when you need it.
